VO₂max and healthy aging: why it matters after 40

TL;DR. VO₂max — the maximum rate your body can use oxygen — is one of the most studied markers in the research on healthy aging and longevity. It declines naturally with age, roughly 10% per decade in untrained adults, and that decline tracks with losing the capacity to do everyday physical things independently later in life. The encouraging part: VO₂max responds to training at any age. Interval training raises it in older adults much as it does in younger ones. After 40, the goal shifts from chasing a personal best to defending and rebuilding aerobic capacity — because in the literature, higher cardiorespiratory fitness is consistently associated with lower all-cause mortality. Why VO₂max is more than a fitness number

For endurance athletes, VO₂max is a performance ceiling. For everyone else, it’s something closer to a measure of physiological capacity — how much aerobic reserve you have to draw on. That reserve matters more, not less, as you age, because daily activities (climbing stairs, carrying things, walking uphill) demand a fixed amount of oxygen, and a higher ceiling means those demands sit at a smaller fraction of your maximum.

In large cohort studies, cardiorespiratory fitness shows one of the strongest and most consistent associations with long-term health outcomes. Mandsager and colleagues, studying over 120,000 people, found higher cardiorespiratory fitness associated with lower mortality, with no observed ceiling of benefit (Mandsager et al., 2018, JAMA Network Open). This is association, not proof of cause — but it’s why VO₂max appears so often in the longevity conversation.

The age curve — and why it isn’t fixed

VO₂max tends to decline with age. In sedentary adults the drop is often cited at around 10% per decade, accelerating after midlife. But a large part of that decline reflects reduced activity, not aging alone. Training meaningfully slows the curve.

The key research finding for anyone over 40: the aging system still adapts. The trainability of VO₂max is preserved into middle and older age — the percentage response to interval training is broadly similar to younger adults, even if the absolute ceiling is lower.

What this means for training after 40

The strategy shifts with age, but the tool doesn’t:

  1. Defend the ceiling. The priority becomes slowing or reversing decline, not setting records. Two quality interval sessions a week is a strong defence.
  2. Respect recovery. Recovery capacity changes with age. More easy aerobic work, more sleep, more space between hard sessions — quality over volume.
  3. Train the recovery side too. Breathwork and easy aerobic work support the parasympathetic recovery that hard intervals stress.
  4. Measure relative progress. Compare yourself to your own baseline, not to a 25-year- old. Heart-rate recovery trending the right way is the win.
